Pilar cysts, also known as trichilemmal cysts or wens, are a type of relatively common cyst found primarily on the scalp, though they may also occur elsewhere on the body. These cysts occur more often in women than men, and may be related to a genetic inheritance. Thirty percent of people have a single cyst and 70% of the time there are multiple on the same person.
Pilar cysts are often firm, slightly mobile and have no punctum or “head”. They are usually asymptomatic, though they may grow in size over time, may become inflamed or infected, or interfere with local hair growth. They are typically benign, though cancerous cells may be present or develop in very rare cases.
Pilar cysts can be easily treated through surgical excision with local anesthesia in the office setting. Patients should consider removal of these cysts if they are painful, inflamed, or rapidly changing.
